One drop, ten seconds, and a number that decides three months of syrup.

"Anaemia hai, doctor. The school camp report says so."

She unfolded a small printed slip and pushed it across the desk, smoothing it twice, the way people do with paper they have been carrying around and worrying about. Her daughter, six, was spinning slowly on the stool, entirely uninterested in her own haemoglobin.

I asked her two things. Did anyone squeeze the child's finger to get the drop out? And what else was written on the report?

She did not remember about the squeezing. And there was nothing else on the report. One number, with three months of iron syrup written underneath it.

Both answers matter. The second one matters more.

Two surveys, one country, very different answers

India has counted anaemia in its children twice over and got two different countries back.

The National Family Health Survey, carried out at enormous scale by teams taking a drop from the fingertip, reported that about 67 in every 100 children under five were anaemic. The Comprehensive National Nutrition Survey, run between 2016 and 2018, drew blood from a vein and sent it to a laboratory. Among children aged one to four it found 40 in 100 (Yadav and colleagues, PLOS Global Public Health, 2024). Across the wider one-to-nineteen group it found 30.7 in 100, less than half the survey figure (Kurpad and Sachdev, Indian Pediatrics, 2022).

Two explanations are on the table for that gap. One is the cut-off itself. Researchers went back to the healthy children inside the venous survey and worked out what haemoglobin actually looks like in a child with no deficiency and no inflammation. Those levels sat one to two points below the numbers we have been using. Applied to all 41,210 children and teenagers with a valid reading, anaemia fell from 30.0 in 100 to 10.8 (Sachdev and colleagues, Lancet Global Health, 2021). The other explanation is what happens to a sample collected in a hurry, in the field, by a team getting through hundreds of children before lunch (Rai and colleagues, British Journal of Nutrition, 2022).

Let me be honest rather than dramatic, because you deserve the honest version. Not everyone agrees about the cut-off. A WHO-funded analysis pooling healthy children from eight countries found that for two- to five-year-olds the line sits almost exactly where it already is, and found no evidence it should differ between ancestries at all (Braat and colleagues, Lancet Haematology, 2024). Below two years, it did come out lower.

"So the finger-prick machine is useless?"

Not at all. I use one in my own clinic, most days of the week, and I trust it.

A capillary sample is only as good as the drop it is taken from. Milk or squeeze a fingertip and tissue fluid comes out along with the blood, thinning the sample and pushing the reading down. Take it from a cold, tightly clenched hand and you can get a falsely high one. But wipe away the first drop, let the next one well up on its own from a warm relaxed hand, and the number tracks a laboratory result closely enough to act on.

So it is not the finger that is the problem. It is the hurry.

And there is a second problem, bigger than the first one. Anaemia is a number, not a diagnosis. In that national survey, only about a third of the anaemia found in children was explained by iron deficiency. Three out of four anaemic toddlers were short of iron, B12 or folate, so for a quarter of them none of the three was the answer. And in this part of the country, a stubbornly low haemoglobin with small red cells is sometimes thalassaemia trait, which no amount of iron will fix.

Which is why the reflex of low number, start iron, repeat in three months quietly misses most of the children it is meant to help, and gives iron to some who never needed it at all.

Lemon on the dal, guava after lunch. Much of the answer is already on the table.

What I actually ask parents to do

Do not start three months of anything on one number. A careful repeat, or a complete blood count from a laboratory, costs little and settles whether there is anything to treat. This is my own practice rather than a national rule.

Ask for the whole report, not just the haemoglobin. The size of the red cells sits in the same printout and is the first clue to what is actually going on.

If iron is started, fix a date to check it. A genuinely iron-deficient child's haemoglobin climbs within four to six weeks. If it has not moved, the answer is not a bigger dose. It is a different question.

Put a sour thing next to the iron-rich thing. Lemon on the dal, guava or amla after lunch. And keep tea and a large glass of milk away from meals, because both block iron absorption at exactly the wrong moment.

The mother that morning had done nothing wrong. She had taken a piece of paper seriously, which is what a good parent does.

A single number is a question, not an answer. Ask the next one before you open the bottle.

"Is It the Mobile, Doctor?" Why Your Child's Eyes Need Sunlight More Than a Screen Ban

Indian children playing hopscotch in a residential lane in golden late-afternoon sunlight

Two hours of this is the best prescription I can write.

"Doctor ji, she has started sitting right in front of the TV."

The mother says it. Then she looks at her daughter. Then back at me, half worry, half apology, as though the sitting-close were somehow her own fault.

And I already know the next question. It is always the next question.

"Is it the mobile?"

I have a soft spot for that question, because whoever asks it has already done the difficult part. They noticed. But my answer is rarely the one they came in expecting.

The most useful thing you can do for your child's eyes has very little to do with the phone. It has to do with opening the door and sending her outside.

Something changed, quietly

Think back to your own childhood. You went out after school and came home when the streetlights came on. Nobody scheduled it. Nobody called it "outdoor time."

Here is what none of us knew then: those afternoons were treatment.

A team at LV Prasad Eye Institute tracked city children in India. In 1999, about 4 in every 100 were short-sighted. By 2019, it was 21 in every 100 — roughly one extra child per hundred, every year, without pause. If that line holds, close to half of urban Indian children will need glasses by 2050 (Priscilla and Verkicharla, Ophthalmic & Physiological Optics, 2021).

Half the class. In one generation.

Let me be honest rather than dramatic. A newer review of Indian studies came out lower, around 8 in 100, with no clear year-on-year climb (Kumar and colleagues, Ophthalmic Epidemiology, 2025). Our data is uneven. But ask any of us in a city clinic and you hear the same sentence: we are writing far more prescriptions for far smaller children than we used to.

"So what, doctor? She'll just wear glasses."

Usually said by a father, usually kindly. And it is fair — glasses are not a tragedy.

But short sight is not a problem with the glass. It is the shape of the eyeball. The eye grows a little too long from front to back, like a soft ball being gently stretched.

And that stretching does not come back. Not ever. A stretched eye has thinner walls behind, which across a lifetime means a raised risk of the retina peeling away, of damage to the central seeing area, of glaucoma.

Which is why the age it starts matters far more than the number on the prescription. A child who becomes short-sighted at seven has years of growing still ahead, and the number climbs through every one of them. Our job is not to correct the number. It is to push the starting date as late as we possibly can.

The medicine is sunlight

Researchers in Shanghai put light-sensing smartwatches on nearly 3,000 children for a year. Total minutes outdoors mattered less than the pattern: only unbroken stretches of at least fifteen minutes, in genuinely bright light, made any difference (Chen and colleagues, JAMA Network Open, 2024).

Your best-lit drawing room is nothing compared with an ordinary Indian afternoon. You cannot get this dose indoors. Not beside a window. Outside, or not at all.

And here is the part I most want you to carry away. The same team found that in children who had already begun drifting towards short sight, outdoor time helped much less (Chen and colleagues, British Journal of Ophthalmology, 2026).

Sunlight is a prevention, not a cure. By the time she is squinting at the blackboard, the window has largely shut. Which is exactly why I am telling you now, while she is five, and not in five years.

If it has already started we are not helpless — but know what is being oversold. The Cochrane group pooled 104 trials and 17,509 children. Strong atropine drops slow things down convincingly. The very weak drops below 0.1%, which is what most Indian families are actually offered, showed only a small effect, rated very low certainty (Lawrenson and colleagues, Cochrane, 2025). An American trial of 0.01% atropine found it worked no better than drops with nothing in them (Repka and colleagues, JAMA Ophthalmology, 2023).

What I actually ask parents to do

Two hours outside, most days. Cycling, gully cricket, homework on the terrace at four o'clock. In long stretches, not scattered crumbs.

Get her eyes properly examined — a full dilated test, not a chart on a wall — before school begins, and every couple of years after. Children assume the whole world looks the way theirs does.

If glasses are prescribed, check after a month that they are on her face. Not in the bag.

And about the mobile. Close work probably contributes something, but the evidence for daylight is far stronger. If you are waging a daily war over screen time while she spends every hour of sunlight indoors, you are fighting the wrong war.

We spend enormous energy protecting our children from things we cannot control. The air. The traffic. The exam.

This one is in your hands. It is free, and it arrives every single afternoon.

Open the door. Let her come back dusty and thirsty and slightly too pleased with herself.

Charak Clinics Introduces Cutting-Edge Visual Field AI Technology for Eye Care in Chandigarh Tricity

At Charak Clinics, we're proud to be the first eye clinic in the Chandigarh-Mohali-Panchkula Tricity to bring Elisar's AI-powered Visual Field technology to our patients.

Glaucoma is often called the "silent thief of sight" because it can progress with no early symptoms. Catching it early makes all the difference in preserving vision. That's why we've integrated this AI-driven visual field testing into our diagnostic toolkit - it gives us far more precise, reliable detection of glaucoma and other optic nerve conditions than conventional methods.

What this means for our patients: faster, more accurate testing, earlier detection of subtle vision loss, and treatment plans that are tailored to each person's actual risk profile rather than a one-size-fits-all approach.

As I always tell my patients, your eyes don't usually complain until a problem has already progressed - so regular screening matters, especially if you're over 40, diabetic, or have a family history of glaucoma.

If you're in Chandigarh, Mohali, or Panchkula and haven't had a comprehensive eye exam recently, this is a great time to book one.
